Appointments

What do we need from you?
Please bring all of the medications you are taking (in the pharmacy container) with you when you come for your appointment.

Please let us know of any pertinent changes in insurance, employment, change of address, or relationships with other care/service givers.

Please arrive on time for scheduled appointments and cancel, when necessary, with a phone call.

Please follow the treatment plan recommended by your doctor and let us know immediately if you experience any side effects from prescribed medications.

Medical Records

Due to Federal HIPAA Privacy Regulations, a medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider, we ask that you supply us with their name, address, and phone number.

Prescription Refills

We prefer to refill your medications at the time of your office visit. Please bring a complete medication list with you to each appointment.

Please be sure to give us at least 24 hours notice on all refill requests

Chronic or long-term medication refills require an office visit before they may be refilled. If it has been more than 6 months since your last visit, we may not be able to refill your prescription by phone.

Cancellations

If you cannot keep a scheduled appointment, we ask that you give us 24 hours notice. Patient will be charged $25 for missed appointments when they do not call in.

After Hours Emergencies

If you have an urgent matter after hours and need to speak to the physician on call, please call 494-4000 to reach our answering service. A live operator will answer your call and page the physician. If you experience a life-threatening emergency, PLEASE DIAL 9-1-1 IMMEDIATELY.

Patient Billing & Insurance

Unless arrangements have been made in advance, co-payments, co-insurance, and any outstanding balances are expected at the time of service. Patients may be financially responsible for payment of all services, even if their insurance company does not pay. Patient accounts not paid promptly are subject to third party collections and/or legal procedures. For your convenience, we accept Visa, MasterCard, Discover and American Express.

You can also use our secure Online Bill Pay option to pay current statement balances online without setting up an account.

Any check returned from the bank will result in an additional ($20.00) charge that will appear on your account.

Please always notify our office of any change in name, address, phone or insurance information.

Insurance

Prior to your appointment, please check your insurance information so you will be informed about referrals, co-payments, and any deductible required at the time of the visit.

For your first visit, please bring your insurance card and a photo ID and arrive early to complete the necessary patient information forms. You may also download our patient registration forms and bring the completed forms with you when you come for your appointment.

We accept Medicare as well as most commercial insurers, however, please review all insurance information with our staff prior to services being rendered.

Your health insurance contract is between you and your insurance company. Any complaints regarding your coverage should be directed to your carrier.

Self Pay Policy

Uninsured patients will be required to make payment at time of service, at a discounted rate.
